Aktifitas tanggap darurat tersebut diterapkan secara berjenjang (tiered response) oleh ERO yang berbeda, dimulai dari di fasilitas lapangan/site/field (Site Emergency Response Team), ke tingkat Zona atau setara (Incident Management Team), ke Regional atau Anak Perusahaan (Business Support Team), dan ke Subholding Upstream (Crisis Management Team), bergantung pada kompleksitas keadaan darurat. Berikut ini adalah hirarki organisasi tanggap darurat (ERO) Subholding Upstream:exercises are regularly done to evaluate team readiness (ERO), response effectiveness, completeness & performance of emergency equipment, and procedure accuracy.In an emergency, emergency response activities are conducted by the Emergency Response Organization (ERO). These emergency response activities are implemented in a tiered response by different EROs, starting from the site/field facility (Site Emergency Response Team), to the Zone level or equivalent (Incident Management Team), to a Region or Subsidiary (Business Support Team), and to the Subholding Upstream (Crisis Management Team), depending on the complexity of the emergency.The following is the hierarchy of the Emergency Response Organization (ERO) of the Subholding Upstream:Pada tahun 2024, tercatat sebanyak 17 insiden terkait keselamatan proses, yang terdiri dari 6 insiden Process Safety Event (PSE) Tier 1 dan 11 insiden Process Safety Event (PSE) Tier 2. Seluruh insiden tersebut telah ditindaklanjuti dengan berbagai upaya pencegahan untuk meminimalkan risiko dan meningkatkan keandalan sistem keselamatan.Sebagai bagian dari strategi peningkatan keselamatan proses, perusahaan melanjutkan program yang berfokus pada Process Hazard Analysis (PHA), Major Accident Hazard (MAH), serta registrasi Safety and Environmental Critical Elements (SECE) di CMMS. Langkah ini bertujuan untuk memonitor dan mengidentifikasi potensi kegagalan barrier keselamatan, sehingga dapat dilakukan tindakan mitigasi yang tepat sehingga terhindar dari MAH.Selain itu, peningkatan awareness dan kompetensi dalam keselamatan proses terus diperkuat melalui berbagai program, antara lain:%u2022 Computer-Based Training (CBT) untuk memastikan pemahaman teknis yang lebih baik.%u2022 Sesi berbagi (sharing session) terkait pembelajaran dari insiden sebelumnya.In 2024, 17 incidents related to process safety were recorded, consisting of 6 Process Safety Event (PSE) incidents of Tier 1 and 11 Process Safety Event (PSE) incidents of Tier 2. All of these incidents have been followed up with various preventive efforts to minimize risks and improve the reliability of the safety system.As a part of the process safety improvement strategy, the company continues the program that focuses on Process Hazard Analysis (PHA), Major Accident Hazard (MAH), and Safety and Environmental Critical Elements (SECE) registration in CMMS.
